Early Gastric Cancer



Abstract
In Korea, the gastric cancer is the most common malignancy and its incidence and mortality rates are still ranked ahead of all malignancies. But, the prognosis of early gastric cancer is excellent, overall 5 year survival rate of which is 85~95%,
compared with advanced gastric cancer.
This study deals with the clinical analysis of 55 cases of early gastric cancer, which were confirmed by histopathological examination after surgical resection, during a period of 6 years from january 1988 to December 1993 at department of
General
Surgery, Maryknoll Hospital in Pusan.
@ES The results of clinical analysis were as follows:
@EN 1) The incidence of early gastric cancer was 11.3% of all resected gastri cancer. And the incidence was increased over the years.
2) The ratio of male to female was 1.1 : 1, and the most prevalent age group was 6th decede (38.2% and the mean age was 53.
3) The most common symptom was epigastric pain/discomfort(80%) and the most patient had been complaint during 6 months or less
4) The preoperative diagnostic rate were 42.9% by UGI and 75.9% by endoscopy.
5) The size of lesions varied between 0.5cm and 6.0 cm and the range of 1.1 cm to 2.0 cm was the most common.
6) The depth of invasion was limited to mucosa in only 15 caes(27.3%) and to submucosa in 40 cases(72.7%).
7) The most common site of lesion was antrum(80%) and lesser curvature(47.3%).
8) The most common type by macroscpic classification was type Iic(35.5%), followed by type III(16.4%).
9) In histopathologic calssification according to Japanese society for gastric cancer, the most common type was tubular adenocarcimona(67.3%) followed by signet ring cell carcinoma(27.3%).
10) The overall rate of lymph node metastasis was 18.2%, All but one was invaded to submucosa.
11) follow up study was possible in all cases, and only one male patient was died after postoperative 22th month, and so the overall cumulative survival rate was 85.5%.
12) Relations between variable clinicopathologic factors were not significant.
